- Photocopy of your valid identity card or Birth Certificate issued by the Civil Registry, with no more than one year of issuance.
- 2 recent Passport photos (3.5 cm x 4.5cm).
- Photocopy of the expired passport personal information page or “Declaration of theft, loss or retention” form.

If you make the deposit by wire transfer, you must add US$14 for banking service charges. Cash payments are not accepted at the Consulate.
- When the applicant is a minor, the legal representatives must present the Birth Certificate (with less than one year of issuance) or the identity card for minors issued by the Civil Registry, the “Application for a temporary travel document for minors” form signed by both parents, and photocopy of the valid identity document of both parents; the “Authorization for temporary travel document for minors” form (as applicable) signed by both parents. If one of the parents is in Costa Rica, he/she must go to the General Direction of Immigration office, in Costa Rica, to complete the application and photocopy of the valid identity document of both parents.
